Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2 which allows for multiple and varied meta-epidemiological research studies to evaluate the effect of treatment on trials that have different levels of pragmatism and other design features.
Background
Pragmatic studies are increasingly recognized as providing real-world evidence for clinical decision-making. The term "pragmatic" however, is used inconsistently and its definition and measurement require further clarification. Pragmatic trials are intended to guide the practice of clinical medicine and policy decisions, not to verify a physiological hypothesis or clinical hypothesis. A pragmatic study should strive to be as close to the real-world clinical environment as possible, such as the participation of participants, setting up and design, the delivery and implementation of the intervention, as well as the determination and analysis of the outcomes, and primary analysis. This is a major distinction between explanation-based trials, as defined by Schwartz and Lellouch1, which are designed to prove the hypothesis in a more thorough manner.

Truly pragmatic trials should not blind participants or clinicians. This can lead to a bias in the estimates of the effect of treatment. The trials that are pragmatic should also try to enroll patients from a wide range of health care settings, to ensure that their findings are generalizable to the real world.
Finally, pragmatic trials must focus on outcomes that matter to patients, such as the quality of life and functional recovery. This is particularly important when trials involve the use of invasive procedures or could have serious adverse impacts. The CRASH trial29, for instance focused on the functional outcome to compare a 2-page case-report with an electronic system for the monitoring of hospitalized patients with chronic heart failure. Similarly, the catheter trial28 focused on urinary tract infections caused by catheters as its primary outcome.
In addition to these features, pragmatic trials should minimize the requirements for data collection and trial procedures to cut costs and time commitments. Furthermore pragmatic trials should try to make their results as relevant to actual clinical practice as possible by ensuring that their primary analysis is the intention-to-treat approach (as described in CONSORT extensions for pragmatic trials).
Many RCTs which do not meet the requirements for pragmatism but contain features contrary to pragmatism, have been published in journals of different types and incorrectly labeled pragmatic. This could lead to false claims of pragmatism, and the usage of the term should be standardised. The development of a PRECIS-2 tool that provides an objective and standardized evaluation of pragmatic aspects is a first step.
Methods
In a pragmatic research study it is the intention to inform policy or clinical decisions by demonstrating how an intervention can be integrated into routine treatment in real-world settings. This is distinct from explanation trials, which test hypotheses about the cause-effect relationship in idealised conditions. In this way, pragmatic trials could have a lower internal validity than studies that explain and be more susceptible to biases in their design analysis, conduct, and design. Despite their limitations, pragmatic research can be a valuable source of information to make decisions in the context of healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging from 1 to 5 (very pragmatist). Koppenaal and colleagues found that 36% of 89 pragmatic studies were placebo-controlled, or conducted prior to the licensing. The majority of them were single-center. They are not close to the standard practice and are only referred to as pragmatic if their sponsors agree that these trials aren't blinded.
A common feature of pragmatic studies is that researchers attempt to make their findings more relevant by studying subgroups within the trial sample. However, this can lead to unbalanced comparisons with a lower statistical power, which increases the risk of either not detecting or incorrectly detecting differences in the primary outcome. This was a problem in the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not adjusted for covariates' differences at the time of baseline.
In addition, pragmatic studies can present challenges in the collection and interpretation of safety data. It is because adverse events are usually self-reported and are susceptible to errors, delays or coding errors. It is therefore crucial to improve the quality of outcomes for these trials, in particular by using national registry databases instead of relying on participants to report adverse events in the trial's database.
Results
While the definition of pragmatism does not require that clinical trials be 100% pragmatist there are benefits when incorporating pragmatic components into trials. These include:
By including routine patients, the trial results are more easily translated into clinical practice. But pragmatic trials can be a challenge. The right type of heterogeneity, for example could allow a study to expand its findings to different settings or patients. However the wrong type of heterogeneity could reduce the assay sensitivity and, consequently, lessen the power of a trial to detect even minor effects of treatment.

The difference in the primary analysis domain could be due to the fact that the majority of pragmatic trials process their data in the intention to treat way while some explanation trials do not. The overall score was lower for systematic reviews that were pragmatic when the domains on the organization, flexibility of delivery and follow-up were merged.
It is important to remember that a pragmatic trial doesn't necessarily mean a poor quality trial, and in fact there is an increasing rate of clinical trials (as defined by MEDLINE search, but it is neither sensitive nor specific) that employ the term 'pragmatic' in their title or abstract. These terms may signal a greater awareness of pragmatism within titles and abstracts, but it isn't clear whether this is reflected in the content.
Conclusions
In recent years, pragmatic trials have been increasing in popularity in research because the importance of real-world evidence is increasingly recognized. They are randomized trials that compare real world alternatives to experimental treatments in development. They are conducted with populations of patients more closely resembling those treated in regular care. This method can help overcome the limitations of observational research, for example, the biases associated with the use of volunteers and the lack of coding variations in national registries.
Other benefits of pragmatic trials include the ability to utilize existing data sources, and a higher likelihood of detecting meaningful changes than traditional trials. However, these tests could have some limitations that limit their reliability and generalizability. For example, participation rates in some trials may be lower than anticipated due to the healthy-volunteer effect as well as incentives to pay or compete for participants from other research studies (e.g. industry trials). The need to recruit individuals quickly reduces the size of the sample and impact of many pragmatic trials. Some pragmatic trials also lack controls to ensure that any observed variations aren't due to biases that occur during the trial.
The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described as pragmatism. The PRECIS-2 tool was used to assess the degree of pragmatism. It includes areas such as eligibility criteria as well as recruitment flexibility, adherence to intervention, and follow-up. They found that 14 of these trials scored highly or pragmatic practical (i.e. scoring 5 or more) in any one or more of these domains and that the majority of them were single-center.
Studies with high pragmatism scores are likely to have more lenient criteria for eligibility than traditional RCTs. They also include populations from many different hospitals. The authors suggest that these traits can make the pragmatic trials more relevant and useful for everyday clinical practice, however they do not guarantee that a trial conducted in a pragmatic manner is completely free of bias. Furthermore, the pragmatism of trials is not a fixed attribute; a pragmatic trial that does not contain all the characteristics of an explanatory trial can yield valid and useful results.
